Disability

The State Universities Retirement System (SURS) provides disability benefits for eligible university employees. Applications for SURS disability benefits are distributed by Labor and Employee Relations.

To receive a SURS disability application, an employee must schedule an appointment by emailing IHR-LER@illinois.edu or calling (217) 333-3105 and provide a physician’s statement describing the diagnosis and prognosis and stating that the employee must be off work for more than 60 days.
