Program for Employees with Chronic Health Conditions

Well-Being Services offers this program for employees and their spouses and partners who are dealing with chronic conditions such as high blood pressure, asthma, or arthritis.

What is Live Well, Be Well?
Live Well, Be Well is an evidence-based chronic disease self-management program. Participants discover healthy ways to live with their chronic conditions by learning to:

  • Better communicate with doctors, dietitians, or nurses
  • Reduce frustration
  • Fight pain and fatigue
  • Make daily tasks easier
  • Reduce stress and anxiety
  • Make informed treatment decisions
  • Manage medications
  • Break the symptom cycle
  • Take back control of your life

Who is Live Well, Be Well for?
Any employee with a chronic long term condition such as:

  • Arthritis
  • Heart Disease
  • Asthma
  • Lung Disease
  • Emphysema
  • Parkinson’s
  • Diabetes
  • Osteoporosis
  • All Others

Spouses and partners of employees can also participate.

How does the program work?
The course runs for seven weeks and is taught once a week by two trained leaders, both of whom also live with a chronic condition. Sessions run two hours, include breaks, and are highly interactive.
