Below are processes and forms that HR professionals may frequently perform.

Employment Verifications

Former Employees

Employee or Agencies making request: Individuals formerly employed as a faculty, other academic, academic professional, civil service (including extra help), graduate assistant/hourly, or academic hourly employee should send a written request, or this signed release form, specifying the information needing verified to Illinois Human Resources at 52 E. Gregory Drive, Champaign, IL 61820, or fax to (217) 244-7304; or via email to ihr-dataops@illinois.edu.

Please be advised that it can take 10 – 15 working days to process the request.

Requests for information other than salary, dates, percentage of appointment, or title (e.g., performance, job duties, or attendance) should be directed to the unit(s) in which you were employed.

Current Employees

All current university employees must use the University of Illinois Online Employment Verification System. By doing so, vendors such as banks, mortgage lenders, and government approved housing can access your employment information immediately and process your loan application or housing request much faster. Requests for information other than salary, dates, percentage of appointment, or title (e.g., performance, job duties or attendance) should be directed to the unit(s) in which the employee was affiliated.

To access the Online Employment Verification System (EVS):

  • Access the System HR Services site
  • Click "Access EVS" in the Take Action Box
  • Enter your NetID and password
  • Click on the Login box
  • Enter your 4 digit PIN (if you don’t already have one, click on the Your PIN link and enter the requested information. To move between fields, use the Tab key or click your cursor directly into the next field. By striking the Enter key, you will receive an error message and must re-enter the information)
  • Click Submit
  • Select Establish a Vendor Access Number
  • Click Continue
  • Review VAN Requirements and Instructions for Vendor by clicking on the links provided
  • Enter any 4 digit number to be used as a one time access Vendor Access Number; re-enter the VAN number you created for confirmation
  • Select the level of authorization from the dropdown
  • Click Continue
  • Note the web address for EVS on the VAN Created page.
  • Receive email confirmation

Provide the vendor with the EVS web address and VAN so they can access your information immediately. Please inform the vendor that they have only one opportunity to access your information with the VAN provided. They should view and print the information before exiting the system.

Exit Checklist

Exit checklists are required for mhttps://humanresources.illinois.edu/about/centers-of-expertise/hr-data-operations/?et_fb=1&PageSpeed=off#ost employees leaving the University of Illinois (Resigning or Retiring). They should be completed and returned to the employee’s department for attachment to their HRFE (Human Resources Front End) transaction. When leaving the university, it takes approximately 6-8 weeks for all processes to be completed (please inform your outgoing employee(s) of this). Below are some tips and a list of transactions that do not require an exit checklist.

Tips:
  • Electronic signatures are accepted!
  • The exit checklist is unable to be attached after the separation transaction has gone through the first apply, so it must be attached initially!
  • If the employee is unavailable or refuses to sign the exit checklist, the unit must sign and date on their behalf documenting that the employee was unavailable or refused. This is required for auditing purposes.
  • The exit checklist is required for paid Visiting Scholars.
Transactions that do NOT require an exit checklist:
  • Hourlies (Grad or Academic)
  • Extra Help
  • End jobs (one job ending to accept another appointment)
  • NONRs (appointment ends naturally)
  • Unpaid courtesy appointments with an end date (appointment is ending naturally)
    • If the appointment is being truncated earlier than the natural end date, then a resignation letter is required (an exit checklist is not applicable for an unpaid position).

Michelle Althaus

Michelle is the Deputy Director of the HR Data Operations department in Illinois Human Resources. She has 12 employees on her team, and she is the University’s liaison for the State University Retirement System (SURS) and University Payroll and Benefits. Michelle also sits on various committees regarding system changes, leaves, and policies.

Jordan Anderson

Jordan is one of three processors on our civil service team. Jordan processes all types of transactions and leave balances in PEALEAV. He will begin training on disability leaves, prior service requests, and seniority balances and reports.

Becca Beard

Becca is one of four processors on our faculty, academic professional, and graduate team.  Her focus is on all types of faculty and academic processing including retire/rehires and notices of non-reappointments.

Paula Gladney

Paula works closely with the State University Retirement System (SURS) processing thousands of web events for our faculty and academic professional staff, including prior service, and death claims. She also works closely with University Payroll and Benefits on certification, leave, and termination events.

Daphne Heath

Daphne works closely with the State University Retirement System (SURS) processing thousands of web events for our civil service staff, including prior service, and death claims. She also works closely with University Payroll and Benefits on certification, leave, and termination events.

Robbie Henson

Robbie is one of four processors on our faculty, academic professional, and graduate team.  His focus is on all types of faculty and academic processing including retire/rehires and notices of non-reappointments. 

Heather Hendricks

Heather is one of three processors on our civil service team and is a liaison on one of the five matrix teams. Heather processes all types of civil service transactions, disability leaves, leave balances in PEALEAV, prior service requests, and seniority balances and reports.

Autumn Hiles

Autumn is one of three processors on our civil service team and is a liaison on one of the five matrix teams. Autumn processes all types of transactions, disability leaves, leave balances in PEALEAV, prior service requests, and seniority balances and reports.

Elizabeth Johnson Love

Liz is the Manager in the HR Data Operations department in Illinois Human Resources. Liz’s focus is on probationary extension dates, providing monthly temporary upgrade reports to units, reviewing file requests, ensuring compliance with audits, and serving as the main resource for all things civil service. She also supervises 11 employees daily.

Carl Luss

Carl is one of four processors on our faculty, academic professional, and graduate team. Carl’s focus is on employee group changes and graduate transactions.

Craig Smith

Craig is one of four processors on our faculty, academic professional, and graduate team and is a liaison on one of the five matrix teams. His focus is on all types of faculty and academic processing including retire/rehires and notices of non-reappointments.

Tia Trimmer

Tia maintains the Illinois Human Resources Data Operations phone lines and email inbox. She processes unemployment claims, tuition waivers, child of employee tuition waivers, employment verifications, public service loan forgiveness forms, and ANA leave balance adjustments.
